Enrolling with physical paperwork

Step 1

You will need to fill out the Authorization for Participation on the MyChildrensMercy Patients Portal form. The form can be downloaded by:

  • When the forms are processed, you will receive an email invitation to access the patient portal. Accept the invitation.

  • Your child’s birthdate and security questions are needed.
    The account will be created using your details.
